For the clinical cardiac electrophysiology fellowship, please submit the following via ERAS:
- Common application form
- Personal goals statement
- 3 letters or recommendations (one from current program director)
- USMLE transcripts
- ECFMG status report (foreign medical graduates only)
- Wallet-sized color photo
